Philippine nationals who have an expired Philippine passport or who have lost their Philippine passport and cannot wait for the issuance of a replacement passport, can apply for a Travel Document that is valid for 30 days from issuance and can only be used one-way direct travel to the Philippines. The Travel Document cannot be used for travel outside of the Philippines.

GENERAL REQUIREMENTS:

  • Airline Ticket
  • Duly accomplished application form
  • 4 colored photos 2x2 inches with white background
  • Original and Photocopy of Passport or Birth Certificate in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA)
  • Valid Picture IDs
  • Photocopy of visa issued/ work permit/ residence ID card
  • Processing Fee of US$ 30 in cash and in US dollars only
  • Submit an Affidavit stating the detailed explanation why you need to be issued a travel document, must be signed before a notary public, certified by the relevant Association of Notaries Public, legalized by the Ministry of Foreign Affairs and with English translation. Applicant may opt to execute his/her affidavit before the Consular Officer and pay the notarization fee of US$25.
  • In case of lost passport, submit an Affidavit of Loss with detailed explanation on when, where and how passport got lost, must be signed before a notary public, certified by the relevant Association of Notaries Public, legalized by the Ministry of Foreign Affairs and with English translation. Applicant may opt to execute his/her affidavit before the Consular Officer and pay the notarization fee of US$25.
  • If the lost passport is still valid, submit Police Report, legalized by the Ministry of Foreign Affairs and with English translation.
