ANNOUNCEMENTS

Philippine nationals who have an expired Philippine passport or who have lost their Philippine passport and cannot wait for the issuance of a replacement passport, can apply for a Travel Document that is valid for 30 days from issuance and can only be used one-way direct travel to the Philippines. The Travel Document cannot be used for travel outside of the Philippines.

GENERAL REQUIREMENTS:

  • Airline Ticket
  • Duly accomplished application form
  • 4 colored photos 2x2 inches with white background
  • Original and Photocopy of Passport or Birth Certificate in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA)
  • Valid Picture IDs
  • Photocopy of visa issued/ work permit/ residence ID card
  • Processing Fee of US$ 30 in cash and in US dollars only
  • Submit an Affidavit stating the detailed explanation why you need to be issued a travel document, must be signed before a notary public, certified by the relevant Association of Notaries Public, legalized by the Ministry of Foreign Affairs and with English translation. Applicant may opt to execute his/her affidavit before the Consular Officer and pay the notarization fee of US$25.
  • In case of lost passport, submit an Affidavit of Loss with detailed explanation on when, where and how passport got lost, must be signed before a notary public, certified by the relevant Association of Notaries Public, legalized by the Ministry of Foreign Affairs and with English translation. Applicant may opt to execute his/her affidavit before the Consular Officer and pay the notarization fee of US$25.
  • If the lost passport is still valid, submit Police Report, legalized by the Ministry of Foreign Affairs and with English translation.

Applicant may apply for PSA Birth Certificate and Marriage Contract on-line at www.e-census.com.ph.

The Embassy receives and processes applications for ePassports, which in turn is sent to the Department of Foreign Affairs in Manila for printing and issuance. Please allow 6 to 8 weeks for the entire process up to release of passport to applicant.

All Filipino citizens applying for passports with the Philippine Embassy will no longer need to submit photographs, as their picture, together with their fingerprints and signature, will be taken on-site. This means that all passport applicants, regardless of age, are henceforth required to appear personally.

Applicant may apply for PSA Birth Certificate and Marriage Contract on-line at www.ecensus.com.ph.

Per the regulations of the Department of Foreign Affairs (DFA) and the standards set by the International Civil Aviation Organization (ICAO), all non-machine readable passports may no longer be extended beyond October 31, 2015 and must be completely phased out by November 24, 2015.

 

All Filipino nationals holding Machine Readable-Ready Passports (MRRP; green passports) and Machine Readable Passports (MRP; maroon passports) will no longer be allowed to apply for an extension of the validity of these passports after October 31, 2014.

 

PASSPORT

 

They must instead apply for a new e-Passport as soon as possible before the expiry of their current MRRP (green) or MRP (maroon) passports. Those who fail to do so will likely encounter difficulty at immigration checks when traveling through any ports of entry around the world after October 2015.

 

Passport holders are also reminded that there are strict rules for applying for an extension of the validity of expiring or expired passports. The following are the conditions for the extension of validity of ePassports:

 

  1. Death in the family requiring the OFW and members of his/her dependent family to urgently travel to the Philippines;
  2. Medical or Legal emergencies requiring the OFW and members of his/her dependent family to urgently travel to the Philippines or another country for medical treatment or legal proceedings;
  3. OFWs with sufficient proof of such status;
  4. A Filipino Citizen who is a permanent resident of a foreign country who is returning to said foreign country;
  5. An application for ePassport has been filed at the Embassy simultaneous to the application for extension of passport validity; the release of new passport will either be at place of application or at the Embassy nearest the applicant´s place of destination;
  6. Presentation of proof of urgency such as a death certificate, medical certificate, and similar medical documents, subpoena or notices of hearing and the like. OFWs shall also be required to present a valid employment contract processed by the POEA or POLO.
  7. Confirmed flight details and plane tickets
  8. Accomplished Application Form
  9. Processing fee of US $25 payable in cash and in US dollars only.

 

The extension of passport validity is processed on the same day.

 

Important: ePassport validity extension may only be given to expiring and not expired passports.

  • Submit an Affidavit of Loss with detailed explanation on (1)when, where and how passport got lost, and (2) indicate that he/she fully understands that at the moment the passport is declared lost or stolen, it is automatically deemed cancelled and if found it can no longer be used to avoid complications with immigration authorities. The affidavit must be signed before a notary public, certified by the relevant Association of Notaries Public, legalized by the Ministry of Foreign Affairs and with English translation. Applicant may opt to execute his/her affidavit before the Consular Officer and pay the notarization fee of US$25.
  • If the lost passport is still valid, submit Police Report, legalized by the Ministry of Foreign Affairs and with English translation.
  • Photocopy of first page of lost passport (if available)
  • Other Supporting Document as necessary

 

Please take note that there will be a 15-day clearing period, on top of the 6 to 8- week processing period will be imposed if the lost passport is still valid. The 15 day waiting period allows the DFA to verify if the passport is actually lost and has not been used by another. Said period also allows the DFA to report the lost or stolen valid passports to the NBI, Bureau of Immigration, Philippine Foreign Service Posts and Foreign Embassies and Consulates in the Philippines, and the National Central Bureau of the INTERPOL/Philippine Center on Transnational Crime.

Birth Certificate issued by the Philippine Statistics Authority (PSA). Additional requirement for married woman, Marriage Certificate issued by the Philippine Statistics Authority (PSA).

For electronic passport bring: original and photocopy of page 2 only.

4. Photocopy of visa issued/ work permit/ residence ID card and other Identification cards

5. Passport Fee: USD 60

6. For Dual Citizen, present your original copy of Oath of Allegiance.
