ANNOUNCEMENTS

Consular Officers are authorized to solemnize marriages between two Filipinos of the states under the jurisdiction of the Embassy/Consulate General. Both bride and groom must personally appear at the Philippine Embassy to accomplish and sign the Application for Marriage License and to submit the documentary requirements.

 

The ceremony of marriage solemnization will be on a date after the mandatory 10-day publication of the marriage (marriage bann) following the submission of the complete documentary requirements at the Embassy.

 

The contracting parties must provide two witnesses, both age 21 years or older, who will attend the marriage ceremony and sign the Contract of Marriage.

 

Documentary Requirements for the Solemnization of Marriage at the Philippine Embassy

 

Original and a photocopy of each document must be submitted to the Embassy upon application for marriage license.  The original documents will be returned to the contracting parties after solemnization of marriage.

  5. Groom’sCertificate of Live Birthissued by PSA authenticated by DFA
  6. Groom’sCertificate of No Record of Previous Marriage (CENOMAR)issued by PSA, authenticated by DFA
  7. Photocopy of Groom’s Philippine passport
  8. Solemnization Fee: US$60, to be paid in cash and in US dollars only.

 

Additional documents for each personal circumstance that applies:

 

  • For a contracting party who had a previous marriage annulled by court:Amended PSA Marriage Certificatewith annotation of annulment,authenticated by DFA
  • For a contracting party who had a previous marriage dissolved by a Philippine Court recognition of foreign divorce:Amended PSA Marriage Certificatewith annotation of annulment,authenticated by DFA
  • For a contracting party who is a widow or widower:PSA Death Certificateof deceased spouse,authenticated by DFA
  • For a contracting partyage 18-21 years old, an Affidavit of Consent executed by the parents of the contracting party, notarized by a Philippine public notary and authenticated by DFA
  • For a contracting partyage 22-25 years old, written Parental Advice of parents, notarized by a Philippine public notary and authenticated by DFA

 

The PSA issued Certificate of No Previous Marriage, birth certificate, marriage certificate, or death certificate from the PSA (Philippine Statistics Authority, formerly the NSO) must be brought to the Department of Foreign Affairs’ Authentication Division located at:

  • DFA NCR-Northeast (Ali Mall, Cubao, Quezon City)
  • DFA NCR-East (SM Megamall)

  • DFA NCR-South (Metro Gaisano Alabang)
  • DFA Regional Consular Offices (confirm if your local DFA RCO offers authentication, as not all DFA RCOs do)

If a representative will submit and claim the documents at a DFA Authentication unit, the latter must bring these items:

 

  1. A government-issued ID and photocopy of the ID,
  2. Special Power of Attorney signed by the contracting parties and acknowledged by the Philippine Embassy in Argentina; and
  3. Copy of the contracting parties’ passport or ID bearing the signatures of the contracting parties.

Deaths of Filipino citizens abroad must be reported and duly registered. The Philippine Embassy registers deaths of Filipino citizens in Argentina, Bolivia, Paraguay and Uruguay.

Below are requirements for the civil registry death report:

  1. Four (4) copies of the duly accomplished Report of Death form.
  1. One (1) original and three (3) photocopies of the death certificate translated into English and authenticated by the Ministry of Foreign Affairs in the country of death.
  1. Four (4) photocopies of  the passport of the deceased
  1. Payment of Report of Birth Fee of US$25  payable in cash and in US dollars only

The Embassy will forward the Report of Death to the Philippine Statistics Authority (PSA) in the Philippines. You may retrieve the PSA-certified Death Certificate from the Philippines three months after reporting the death at the Embassy.

Note: Consular Mortuary Certificates are issued when remains or the cremated remains of a deceased (Filipino or foreigner) are to be transported to the Philippines

Births of Filipino citizens abroad must be reported and duly registered. The Philippine Embassy registers births of Filipino citizens in Argentina, Bolivia, Paraguay and Uruguay.

Below are requirements for the civil registry Report of Birth:

  1. Four (4) copies of the duly accomplished Report of Birth Form. 
  2. One (1) original and three (3) photocopies of the child´s birth certificate with English translation and authenticated by the Ministry of Foreign Affairs in the country of birth. 
  3. Four (4) photocopies of the passports of both parent/s
  4. One (1) original and three (3) photocopies of parent´s marriage certificate with English translation and authenticated by the Ministry of Foreign Affairs.
  5. Notarized affidavit explaining the reasons for delayed registration, if Report of Birth is filed more than a year after the child is born
  6. Notarized Affidavit of Acknowledgment of the Paternity and Permission to Use Father’s Surname, executed by the father, if parents are not married and the father’s surname is being used by the child
  7. For parents who are dual citizens, four (4) photocopies of Order of Approval or Petition for Reacquisition of Philippine Citizenship, indicating the name of the child as a minor-beneficiary of the parent’s re-acquisition of Philippine citizenship.
  8. Payment of Report of Birth Fee of US$25 and, if applicable, Notarization fee of US$25  payable in cash and in US dollars only

The Consular Officer may require additional proof or documents from applicant to determine the child’s citizenship, identity, or eligibility for registration of birth under Philippine laws.

The Embassy will forward the Report of Birth to the Philippine Statistics Authority (PSA) in the Philippines. You may retrieve the PSA-certified Birth Certificate from the Philippines three months after reporting the birth at the Embassy.

The Family Code of the Philippines (Sec. 664) provides that “records of marriages whether performed by the Philippine Consular Officer or by the local religious or civil authorities shall be kept and transmitted to the Philippine Civil Registrar through the Department of Foreign Affairs”.

 

Filipinos who married in Argentina, Bolivia, Paraguay and Uruguay should file a report of their marriage at the Philippine Embassy in Argentina if they want their marriage acknowledged by the Philippines. The Report of Marriage should be done within 12 months of the date of marriage. If the Report of Marriage is filed after 12 months of the date of marriage, an Affidavit of Late Registration must be accomplished.

 

 

BELOW ARE THE REQUIREMENTS FOR THE REPORT OF MARRIAGE:

 

  1. Four (4) copies of the duly accomplished Report of Marriage Form
  2. One (1) original and three (3) photocopies of Marriage Certificate with English translation and authenticated by the Ministry of Foreign Affairs in the country of marriage
  3. Four (4) photocopies of the passports of both parties
  4. Four (4) photocopies of birth certificate of both parties
  5. Payment of Report of Birth Fee of US$25 ; Affidavit of Late Registration Fee of US$25

 

ADDITIONAL REQUIREMENTS

 

For contracting parties of the marriage who were previously married, the following additional documentary requirements must be submitted:

 

a. If either the groom or bride is a Filipino who had a previous Philippine marriage annulled, three (3) photocopies of the PSA-issued Annotated Marriage Certificate which indicates the details of the annulment. Proof of the authenticity of the amended marriage certificate must be shown, either the Official Receipt issued by the PSA for the amended marriage certificate or Certificate of Authentication issued by the Department of Foreign Affairs of the Philippines.

 

b. If either groom or bride is non-Filipino divorcé, three (3) photocopies of the final decree of divorce.

 

c. If either groom or bride is a widowed, three (3) photocopies of the death certificate of the deceased spouse.

 

IMPORTANT INSTRUCTIONS IN FILLING UP THE REPORT OF MARRIAGE FORM

 

  • For the field of Wife’s Last Name, please write the wife’s maiden Last Name (or her Last Name before the marriage)
  • Please leave the Date of Report and ID# fields blank. The Embassy will fill up this part.
  • For all fields requiring dates, please use the dd-MMM-yyyy format (e.g. 23-FEB-2012, 1-NOV-1984).
  • Please do not forget to affix by hand the signatures in the fields provided for Signature of Husband and Signature of Wife and the date and place of signature at the bottom of the form.
  • The Embassy’s Report of Marriage Form (PDF format) is now fillable. You may enter information as needed and then print out the form with the entries typed-in. You need to print out the form after filling it in.
  • If filling the form by hand, please write legibly and please use BLOCK CAPITAL letters.

 

The Embassy will forward two copies of the Report of Marriage to the Philippine Statistics Authority (PSA) and Department of Foreign Affairs (DFA) in the Philippines. Another copy will be kept as part of the Embassy’s records and the third copy will be given to the applicant. You may retrieve your PSA-certified Marriage Contract from the Philippines within three to four months after filing the Report of Marriage at the Embassy.

CIVIL REGISTRY (MARRIAGE, BIRTH AND DEATH)

 

The Philippine Embassy performs civil registry services. Births, marriages, and deaths of Filipino citizens abroad must be reported and duly registered. It is a continuous, permanent and compulsory recording of vital events occurring in the life of an individual such as birth, marriage, and death, as well as court decrees, and legal instruments affecting his civil status in appropriate registers as mandated by Act No. 3753, the Civil Registry Law.

 

Civil Registry Documents establish the occurrence of birth, death, or marriage and therefore provide prima facie evidence of facts surrounding these events.

 

Birth records present many facts about an individual such as the person’s name, date and place of birth, parents, religion, citizenship, among others. The marriage certificate establishes a change in civil status and the legal spouse while the death certificate records the passing away of an individual.

 

These records are important in legal and personal transactions, applying for jobs, obtaining passports for travel, entrance to school, claiming insurance benefits, and others.

 

These records provide data on the number of births, deaths, marriages, fetal deaths, adoptions, and the like, which in turn are essential in development planning.

 

The Philippine Government agencies that are authorized to record changes in the status of the Filipinos are the Philippine Statistics Authority (PSA), Philippine Embassies, and Philippine Consulates General.
